New Patient Paperwork

Completing your intake forms

SpineCo: Chiropractic & Performance uses Jane App as our online booking and patient intake system. New patient intake forms are completed through Jane App when you schedule your first appointment.

You can schedule online and access your intake forms at spinecochiropractic.janeapp.com, or call our office and a team member will walk you through the process.

Forms available through the intake process

Depending on the nature of your visit, your intake package may include a general new patient registration form, a review of symptoms, and a patient history questionnaire.

Patients seeking care related to an auto injury will complete a separate auto injury patient intake form.

Pediatric patients will complete a pediatric-specific registration form.

All forms are provided and submitted securely through the Jane App platform. No printing or faxing is required.

Before your first appointment

We recommend completing your intake forms before arriving so your first visit can focus entirely on your assessment and care. If you have questions about which forms apply to your situation, please contact our office.
